The North Carolina Department of Public Safety (NCDPS) is using technology to further improve reentry services for the formerly incarcerated.
The NCDPS Reentry Programs and Services section is tapping into a statewide coordinated care network called NCCARE360, powered by Unite Us, a technology company connecting health and social care. Having access to NCCARE360 will allow reentry professionals around the state to better locate needed services for the formerly incarcerated. Those services such as locating housing, employment, and transportation, among others, may now be found through this network.
Unite Us powers this secure, coordinated care network for health and social care providers, and community-based partners can work across sectors to identify the needs of individuals such as reentry professionals, and connect them to appropriate resources. The NCCARE360 network also provides a closed-loop process to securely track the referrals and outcomes of the client's health journey. NCCARE360 was originally created for the North Carolina Department of Health and Human Services.
There are a number of advantages to using NCCARE360. The network operates in real-time and allows staff to efficiently send and receive secure referrals, access up-to-date and vetted health and social care resources, and have the ability to immediately contact providers in the formerly incarcerated individual’s local community. Before NCCARE360, reentry resource lists were maintained on static web pages that needed to be manually updated when new services were added.
“Connecting people with the community assistance they need can sometimes be a challenge,” said Nicole Sullivan, deputy secretary for Analysis, Programming, and Policy. “NCCARE360 brings all those community resources together in one network. It will provide reentry professionals and case managers with a one-stop-shop to assist their clients and improve our ability to help people with their needs.”
“Our collaboration with Division of Adult Correction will help formerly incarcerated individuals reenter society, as we continue to expand the network to support individuals and connect them to much-needed resources,” said Danika Mills, state network director, Unite Us. “This partnership aligns with our collective goal to provide equitable access to care, no matter who you are or where you live,” she added.
Agencies participating in this effort include DPS' Reentry Programs & Services section, Division of Adult Correction, Division of Juvenile Justice and the 17 Local Reentry Councils across the state.
